By August 3, officials had documented 3,874 confirmed cases and 1,751 fatalities. This marks Congo’s largest recorded Ebola epidemic and ranks as the second largest globally. The country reached 1,000 cases within just 40 days of initiating its response efforts, whereas a major outbreak that began in 2018 took approximately 235 days to surpass that number.

Health authorities declared the outbreak on May 15 following laboratory detection of the Bundibugyo virus in Ituri province. Subsequent investigations revealed that infections had already been occurring months earlier near Mongbwalu. Initial symptoms in early patients often resembled malaria and other common illnesses. Early laboratory testing primarily targeted the well-known Zaire Ebola species, which delayed recognition of the true outbreak. This delay allowed the virus more time to spread within households, clinics, mining communities, and trading networks before testing and isolation measures were expanded.
The presence of the Bundibugyo strain has also restricted the available medical countermeasures. Approved Ebola vaccines and antibody therapies are designed to target Zaire ebolavirus, responsible for Congo’s 2018 to 2020 outbreak. Currently, no licensed vaccine or proven specific treatment exists for Bundibugyo virus disease. Medical teams rely on rapid testing, isolation protocols, supportive care, infection prevention, and safe burial practices. Although the World Health Organization has supported the enhancement of diagnostic capacity and treatment research, these efforts only began after the virus had already spread across multiple regions.
Delayed detection hindered contact tracing efforts
The epidemic has extended beyond Mongbwalu into numerous health zones throughout eastern and northeastern Congo. Ituri remains the epicenter, but cases have also emerged in North Kivu, South Kivu, Haut-Uele, and Tshopo. By July 30, response teams had monitored 17,863 contacts. However, follow-up coverage varied widely, particularly in areas affected by insecurity and logistical challenges. Additionally, many new patients have been identified outside known contact lists, indicating that surveillance efforts have yet to fully trace every transmission chain.
Ongoing conflict has complicated case identification and patient management. Attacks by armed groups have obstructed roads, disrupted health activities, and caused some teams to suspend field operations. High mobility among populations—especially around mining sites, markets, towns, and displacement camps—further hampers contact monitoring. Healthcare facilities are also grappling with shortages of protective equipment, trained personnel, transportation, and laboratory access. As of July 30, Congo reported 151 health worker infections and 44 deaths, adding additional strain to an already overwhelmed response system.
Insecurity and vaccine gaps fuel faster transmission
Ebola primarily spreads through direct contact with the blood or body fluids of an infected individual. The danger is heightened in homes, clinics, and burial sites lacking rigorous infection control measures. Over 60% of recent fatalities have occurred outside treatment facilities, complicating safe burial efforts and contact investigations. To combat this, Congo’s health ministry, the World Health Organization, and Africa CDC have expanded laboratory capacities, treatment centers, border controls, and public awareness campaigns. Despite these efforts, the pace and geographic reach of new infections have outstripped response capabilities.
Uganda successfully contained its linked outbreak on July 28 after 42 days without new cases. France also reported no secondary transmissions stemming from a single treated case. Nonetheless, Congo remains the focus of persistent transmission, with a death rate approaching 45% as of early August. The outbreak’s rapid progression is attributed to delayed detection and the absence of strain-specific vaccines and treatments. Challenges like missed contacts, ongoing conflict, staffing shortages, and population movement have contributed to the widening scope of transmission. These factors set the current Bundibugyo epidemic apart from previous Ebola outbreaks in Congo.
